Free workshop on how and why to complete advanced care plans and forms for end-of-life care. This part of end-of-life planning outlining your wishes for the scope of physical, emotional and medical care that you would like at end of life and the documents that help you communicate that to those who would facilitate that care. Also good for care-givers.
